Frenectomy by A.R.C Laser : Dr Sheril Aida

The prominent flesh or frenum that resembles Nurul's case.

With the purpose of achieving perfect smile, aesthetic concerns have led to an increasing importance in seeking dental treatment. Last few weeks, a patient named Nurul came with her friend to see me for dental consultation. She complaint of prominent flesh on her upper gum. She was worried and told me that if possible she wants to look good on her wedding day as she is getting married in two months time. For the longest time, she was unsatisfied with her smile but did not know how to fix it. She was also concern about the big gap in between her upper two front teeth.

“I really wanted to have a smile that would allow me to smile my widest and happiest on my wedding day, Dr Sheril”, says Nurul.

Based on my examination, I noticed that the prominent flesh that she was mentioning about was a frenum. A frenum is a fold of tissue in the mouth. Frenums are usually found between the upper two front teeth, under the tongue and on the sides of the gums.

Before Frenectomy : Prominent gum tissue noted between Nurul's upper two front teeth

In Nurul’s case, the frenum in between her upper front teeth was too tight causing a pull on her gum leading to median diastema which is the midline gap between her upper front teeth.

A frenum has no purpose and removing one causes no loss of function. So I suggested her to undergo a procedure called Frenectomy which is a simple oral surgical procedure that eliminates the presence of a frenum in the mouth. Frenectomy basically can improves ones facial appearance such as in Nurul’s case.

At first, she was afraid to undergo the surgery because it sounded scary. As I explained to her that instead of removing the frenum using the traditional way by means of a scalpel/blade, we will be using A.R.C Laser to remove the gum tissue.

After Frenectomy : Attachment of the gum has been removed.

“Not to worry Nurul, frenectomy is a simple and fast surgery, so you can just sit back and relax during the procedure”, I reassured her. The surgery can be done in as little as 10 to 15 minutes.

“Plus, you don’t even need stitches as we are using laser. Thus, there will be less pain, lower risk of postoperative infection and a faster healing”, I added.

A frenectomy generally heals without problems and with little pain. It usually helps to take over-the-counter nonsteroidal anti-inflammatory drugs, such as ibuprofen to relieve the pain. Rinsing with salt water helps keep the area clean.

2 Weeks After Frenectomy : The wound has healed.

Two weeks after the surgery, Nurul told me that she was happy with the result. During the follow up visit, the wound has healed and due to time constraint, I only managed to do a minor makeover on her upper front teeth using a composite veneer.

“Thank you Dr Sheril”, says Nurul.

“You are most welcome! Do not be thrilled just as yet as the best part has yet to come”, I replied.

This is just a minor makeover. We will be closing the gap completely after this and I could see the excitement in her face as she walks out the room.
